About Dr. Luigi Mangiacotti

Dr. Luigi Mangiacotti, DO, is a board-certified neurologist licensed in California and Nevada. He is preparing a direct-pay telehealth practice for adults with migraine and other primary headache disorders.

Dr. Mangiacotti completed his neurology training at Cedars-Sinai Medical Center in Los Angeles and earned his Doctor of Osteopathic Medicine degree from Western University of Health Sciences–COMP in Pomona, California.

In addition to his telehealth work, Dr. Mangiacotti currently practices as an inpatient neurohospitalist, caring for patients with a broad spectrum of acute neurologic conditions. He also serves in an academic teaching role, working with trainees and emphasizing careful bedside reasoning, diagnostic accuracy, and evidence-based care.

His approach to headache care is thoughtful and thorough, with a focus on careful listening, clear recommendations, and practical treatment plans tailored to each patient.

This practice is intended for adults with selected outpatient headache concerns when telehealth is appropriate. Visits are planned for patients physically located in California or Nevada at the time of future care.

Why is this a direct-pay practice instead of an insurance-based practice?

This practice is designed as a simple direct-pay telehealth service. By not billing insurance, I can offer transparent pricing, a more streamlined patient experience, and a direct physician-led approach without many of the administrative delays and restrictions that often come with insurance billing. If appropriate, patients may be able to use out-of-network benefits, but coverage and reimbursement vary by plan.

How is this different from larger telehealth platforms?

This practice is designed around continuity, focus, and clarity. Rather than a large telehealth platform where patients may see a different clinician each time, this is intended to be a physician-led practice centered on ongoing headache care. Follow-up visits are designed to build on prior visits, prior treatment decisions, and your individual history.

When are follow-up visits usually scheduled after launch?

Follow-up timing depends on your symptoms, treatment plan, and how recently medications were started or changed. Many patients are seen again within about 4–8 weeks after an initial visit or medication adjustment.
